Evaluate and arrange for treatment (please browse with Google Chrome) through the “Platform for self-assessment and community outpatient appointment for COVID-19 patients”

Step 2:

If you are diagnosed as a confirmed patient, you need to fill in the following declaration form:

Declaration Form for COVID-19 Confirmed Cases (Staff and other Personnel)

Step 3:

Call the COVID-19 infected hotline (28 700 600) if needed.
